On behalf of East London Foundation Trust (ELFT), Compass Wellbeing CIC invites local voluntary, community, and social enterprise organisations to come forward, as part of ELFT’s continuing Community Mental Health Transformation Programme, to express interest in providing a specialist older adult community connector.
Successful applicants will be invited to work with East London Foundation NHS Trust to continue the development of the new model of care. Over the past three years ELFT has worked with a range of providers and partners in co-designing, testing and supporting the community connector role. The role is a core member of the Neighbourhood Mental Health Team operating in the three east London boroughs and has been essential in supporting the mental well-being of people with complex mental health and social needs.
ELFT is committed to developing this role and is actively considering ways to increase the number of posts in future (this will be dependent on local and national funding opportunities). As such, there is a possibility of additional roles being created which could either be procured through a separate process or as contract variations through this process.
Due to the localised nature of the role we ask that providers apply for provision within Primary Care Networks 1 & 2 and they have experience of working with the older population of Tower Hamlets.
